Summer Summits 2019

The TCPi Summer Summits are special events to celebrate our practices’ transformation journey over the last three and a half years. Held in each region, attendees include practices that have been involved, quality improvement advisors who have helped deliver QI interventions, and TCPI leadership. The Summer Summits offer participating practices a chance to see the leadership involved, to tell their stories of the excellent transformation work they have done, and to share their excitement about the continuing journey to value-based care after the TCPI project comes to a close. We’ve collated some of the key materials and highlights from the Summits, and if you’d like more information please contact midsouthptn@vumc.org.

MidSouth PTN Accolades

Dr. Robert Flemming attends MidSouth PTN Summer Summits, praises transformation success

Dr. Robert Flemming, Director of the Transforming Clinical Practice Initiative at the Centers for Medicare and Medicaid Services joined us for our Summer Summits. Our bright spots and take aways include: Vanderbilt Health Affiliated Network successfully transitioned 15 practices to APMs and transformed 30 practices beyond TCPi Phase 3. Baptist Memorial Health Care successfully transitioned 6 practices to APMs and transformed 17 practices beyond TCPi Phase 3.

“A primary care practice achieving a 65% reduction in opioid prescribing rates within four months. A new hydration campaign at one women’s health practice reducing emergency room (ER) visits by expectant mothers by 4.3%.
